The Miami-Dade Sheriff's Office said there was a dispute between people in the establishment when a gunman shot an employee and a patron.

An arrest report identifies the suspect as Paul Edward Koch, 62. He allegedly got into a physical altercation at the bar when he was escorted out by staff, but then returned with a handgun.

The first victim asked him to leave, but Koch fired "multiple times, striking and killing" him, MDSO said.

Then, Koch allegedly went to the "patio/outdoor bar area, where he then shot and killed" the second victim.

The sheriff's office said a third man accidentally shot himself in the leg while trying to escape the gunfire "during the chaos." He was rushed to the hospital as a trauma alert, and video of the aftermath showed a rescue helicopter at the scene.

Koch was arrested and charged with two counts of second-degree murder.

A member of the band Airplane Mode said they were playing in the bar when there was an argument over a spilled drink.

"We were playing and apparently a fight broke out, and then the owner of the place kicked out the guys that were fighting, and one of them apparently went to his car, grabbed a gun, come back in and started shooting," Carlos Quijano said.

Video shared by a source close to the investigation shows the moments before the deadly shots were fired. In the footage, men appear to be holding down Koch and restraining another man during a heated confrontation.

Quijano said earlier in the night, the shooter had bought him a drink, and he hadn't noticed anything unusual about him.

"He seemed normal, he'd come and ask for a song, we'd play a song for him, and he tipped us and bought me a drink," Quijano said. "In the beginning, early in the evening, he was just another customer, it was so crazy."

Authorities have not named the first victim, but friends said he was a barback at Club Glades. The second was identified only by his last name, Hess. Friend Nicole Terrell said his full name was Marc Hess, and that he had been at her home just hours before the shooting.

"It's unbelievable that it happened to him," she said. "We're just driving by to see what had happened because we've been up all night upset so, just praying for his family."

Terrell remembered Hess as a kind father and musician.
